Electronic Signature Consent & Disclosure

- Legal effect. Your electronic signature constitutes a legally binding signature with the same force as a handwritten signature under the E-SIGN Act (15 U.S.C. § 7001) and applicable UETA law.
- Consent to sign electronically. You consent to conduct this transaction electronically and to receive this application in electronic form.
- Authority to sign. You represent that you are duly authorized to bind the Applicant named in Section I.
- Retention. You may print or save a copy for your records. A paper copy can be requested from Maxtel at no charge.
- Withdrawing consent. You may request a paper form instead by contacting your Maxtel sales representative before signing.
- Audit trail. Maxtel records the typed name/initials, method, and timestamp as part of this application's audit trail.
